• PLEASE do not contact the CCS office with questions--contact your athletic director or administrator at your son or daughter's school.  They are better able and are the persons responsible to, answer your questions.  If they do not know the answer, they will contact the CCS.  Parent e-mails and phone calls to the CCS will not be returned, they will be referred back to the school.
• Parents should expect to need assistance from your school Athletic Director or Athletic Secretary or Administration at your school to understand and complete this application. 
• Arriving at any school or the CCS office unannounced in person and expecting to meet with personnel to review or discuss or complete eligibility forms is an inappropriate expectation. 
  • Parents should NOT expect to drop in to a school to get any signatures and walk out with the form that same day!
